Consider purchasing Student Select, an inexpensive plan designed to cover college students studying in a University or Accredited College away from home. Individual Health Insurance
You can apply for Blue Cross Aware Care coverage by simply completing the application. Access the application by pressing the button below, complete, then print, sign and send to our office with your first month’s premium, payable to “BCBSM”. Please consult with Janice Farris, our resident Life/Health Agent first, to determine which plan best suites your needs.
Employer Sponsored Plans
If you are a current Blue Cross group, you can use the application below to add new employees. Simply have your employee complete the application or print and complete by hand, sign and send or fax to our office.
If you are a new group wishing to consider coverage, please contact Janice Farris, Life/Health Insurance Agent for competitive quotes. If appropriate, you can use the application below to apply for rates from Blue Cross. Submitting an application does not obligate you to purchase, however, it will provide you with the rate for which your group is eligible.
