Mortgage Pre-Approval Checklist

If you would like to request pre-approval for a mortgage loan, contact us for a kit, then complete, sign, and return these forms (which will come with your kit):

• Residential Loan Application
• Borrower's Certification and Authorization
• Notice About Appraisal of your Property
• Servicing Disclosure Statement
• Homeowners Insurance Information Request

In addition, please include the copies of the most recent information for each of the following:

If employed:

• Paystub(s) documenting one month pay history
• W-2 forms for the past 2 years.

If self-employed:

• Personal tax returns for the past 2 years
• Business tax returns for the past 2 years
• Bank statements (all accounts) for 2 months
• Statements for stocks, bonds, retirement accounts.
